For patients living with a life-limiting illness that prefer to be cared for in their own home, then our Locality Care Teams can help with this.
Our Locality Care Team works alongside community nurses, GPs and other health professionals to help improve levels of comfort and well-being for patients and avoid unwanted hospital visits.
The team consists of trained nurses, therapists and health care assistants with specialist end of life training and experience. The team provides short-term care (for up to three months) between 8am and 8pm, 7 days a week.
