Farleigh Hospice is a high performing organisation providing specialist palliative and end of life care for people with life limiting illness in the Mid Essex region. Our team at Farleigh has grown and evolved to meet the changings needs of the community we serve and we are looking for highly motivated Motor Neurone Disease Co-ordinator to join our clinical team.

As a Motor Neurone Disease (MND) Co-ordinator you will work within a multi-disciplinary team (MDT) providing specialist advice, information and supportive care to hospice patients living with MND and their families, liaising and referring on as necessary to appropriate health and social care professionals.

As the Farleigh Hospice MND Co-ordinator you will support patients and carers in their own homes, in the Inpatient unit, at outpatient appointments and within a group setting. You will be empathetic with excellent communication skills and be confident to engage in difficult conversations.  

You will be a proactive problem solver with a “can do” attitude.

 

You will have established administrative skills and be able to organise MDT meetings across service areas. You will provide expert information and resources on the care of those living with MND to colleagues and other health and social care colleagues. You will participate in building and maintaining strong relationships with external partners and stakeholders to deliver the best possible coordinated care for patients living with MND within Mid Essex.
