ROLE

Your role is to provide counselling to the bereaved family members (and other significant people) of patients who have died whilst under the care of Farleigh Hospice and individuals referred externally to CIRCLE adult bereavement support.

 

KEY TASKS

Tasks may include (not exclusively)

*                Undertake relevant therapeutic casework as requested by the Head of Department/Team Leader/Counsellor/office co-ordinator

*                Make regular contact with your allocated case-load

*                Undertake bereavement assessments for referrals received into Farleigh as required

*                Manage your case-load in liaison with the Head of Department/Team Leader/Counsellor (cases to be closed or re-referred as appropriate)

*                Demonstrate that you have independent supervision in place [accredited by the relevant regulatory body] to support your clinical practice with Farleigh Hospice clients

*                Attend group operational meetings and annual reviews with the Service Manager/Team Leader/Counsellor

*                Complete online ‘contact sheets’ for statistical purposes on a monthly basis

*                Complete client notes after each session, using the online notes system

*                Assist in the facilitation and delivery of bereavement groups aligned to the core bereavement offer

 

OTHER REQUIREMENTS

 

*                Qualified to Diploma Level 4

*                To adhere to your counselling body e.g. BACP or equivalent, code of ethics

*                To adhere to BACP or equivalent minimum level of supervision

*                To provide documentation as requested by the Bereavement Service Manager/Team Leader, e.g. BACP membership card, certificates of qualifications

*                Maintain strict confidentiality and compliance with GDPR, adhering to data protection policies and procedures at all times

*                Participate in mandatory training and attend any courses that may be relevant/required to the role and department

*                Work to Farleigh Hospice values (we are caring, respectful, dedicated and innovative)

*                Be aware of, and work within, the hospice policies/procedures and boundaries

*                Physical ability to fulfil the requirements of the role including safe moving and handling
